I believe it will be alright, just try to get high quality cat5 cable. USB is 4 lines including ground.
How important is it to use shielded cable for a USB connection? I want to make an extension to the front of the car. This isn't for high data rate either....just for my D-Link USB radio. Will using cat 5 be sufficient? P.S. how many wires in a usb cable? 4+ground?

I would recomend using one of these to extend your usb. they are guaranteed to work and they are pretty cheap (I have 3 myself).

I don't want to get one of those because my cable length is only going to be about 20' max and like i said there isn't going to be much data going back and forth on the connection, so i think it's a waste of money.
If I was you I would get an ordinary 5m USB extension cord. You will get a bit above spec (total 6m or so) but I think that's better than using other cable.
Regarding your primary question. For in-car applications shielding is definitely important, much more than in your home. This because high currents flow everywhere in your car, there can also be ignition noise.
BTW. The data rate on the USB is not determined by the application but by the standard (communication takes place at 12Mbps if you use it or not).
