There are lots of places you can get silent heat sinks and near silent cpu fans+heatsinks. I think the fans are around 18Db's which is nearly impossible to hear anyway. I think they start at like $30ish.
As Slashdot reported, there's a new shuttle out, which I didn't really give a crap about. But look on the second page at their completely silent CPU heatsink.

I THINK the human ear can not hear any thing below 20Dbs (20 vibrations a second). So, yes you can not hear an 18Db fan.

They have silent cpu fans for around $35. I'd recommend and use one of these in my truck. Mainly because the stock heat sink and fan you get with a CPU is very very cheap and My truck gets hot in the summer and i ant something that won't die on me or begin to slow down.
You can also get good heatsinks without a fan depending on how fast and what type your CPU is.
That site i posted above is great for silent anything I also use their silent case fans and silent power supply. The cpu fan and PS have always been the loudiest for me in the past. My hearing might not be as good as other people but when my computer is on in my truck i don't even notice its on. Its right behind me too.
We're coming at this from 2 different sides. I don't care how quiet it is, I was just interested in how well that heatsink and heat-exchanger cooled the CPU. Have you seen those coolers that are on the new Shuttle sold anywhere?
Coolermaster makes the heatpipes those are really good and do a damn good job. I'm not sure where you can get it with the radiator. I've never seen that before.
