What do I need to make a skin from scratch?

Ok so I've allready made 1 skin the easy way, by editing the jpg files of an existing skin.
Now I want to make a skin from scratch. I need a bigger OSK!

Which tools do I need for that? Is there some kind of skin editor, or is everything done in textfiles? Help!

Do I change the mapmonkey skin with the Roadrunner Editor?!
What I want to do is to make the Mapmonkey skin match my Roadrunner skin. The current osk in MM is way to small, so I made a new one, but I don't know how to edit the position of the buttons in MM.
