The red wire is +5v and the black is ground. The flat cable is the SATA cable. So you connect red-to-red and black-to-black using an available power connector from your PSU.

this looks like it is for a slimline sata drive as there are no 3v or 12v lines on the cable

and
pin 4 Manufacturing Diagnostic
mayby pin 1 with 5V and pin 4 with ground ???

No, that's probably a quick way to fry your drive. DO NOT connect pin 1 or pin 4 to anything. Pin 1 doesn't say anything about 5v, and pin 4 says nothing about ground, why would you even think this is correct?
Virac is correct, pin 1 is the key-side.
