Anyway, perhaps it could be due to a spike on the power line? Did the underside of the pcb accidentally touch a metal casing? Anything along those lines would explain it.

Well, all the voltages were right and everything was fine in the house today, so I took the power supply out to the car to test.
Car voltage: 13.8V
3.3V out: 13.8V
5V out: 13.8V
12V out: 13.8V
****.
What happened? I took it back in the house and nothing works anymore. The maxim chips aren't regulating at all. Output = Input.

Check your ground! When the maxim chips are not grounded they pump the input straight through to the output.

Actually, Ill add to that. When the maxim chips are not powered on they pump the input straight through to the output. So it could be either your ground or the supply line to the maxim chips, though as its affecting all of them it it sounds more likely to be the ground.
