Mastero Shutdown controller wiring instruction

Can some one help me wire this thing up. There is no label or anything on the PCB. No pre-wire color code so I have to attach the pic.
Which connection go to which (I numbered the terminals).
This thing is still in the bag Mastero sent. I am going to hook it up with the Sproggy MK3. Mk3.5 later.
Mastero? Anyone?

Mastero, I have questions regarding to this SDC.
The +5V wire is connect to the mobo ATX switch. Is it the pin that show +5V when the mobo is on right (maybe when it off)? I haven't check the mobo pins yet.
If the ACC line go off for a moment (when crank the ACC cut off), Does the SDC go to the shutdown sequence? If that the case, I have to use my old method to combine the ACC and the IGN wires together.

Is this the old style shutdown controller circuit, it looks like the one I have.

IF POWER IS REMOVED ON THE ACC LINE THEN THE SDC WILL WAIT 0~15 MIN BEFORE SENDING THE ATX PLUSE. AND IF WITHIN THAT 15 MIN IF THE ACC LINE IS HOT AGAIN THEN NOTHING HAPPENS

SO WHEN YOU CRANK THE ACC GOES COLD FOR A FEW SECONDS THE SDC WILL NOT SEND THE ATC PLUSE.
