WTB: USB2.0 4 port hub (externally powered)

The cold seems to have destroyed my hub, so I need a new one.

It needs to run on 5v, 7v, or 12v and cost $10-15 shipped

model numbers and/or pics of it are needed (I need to make sure it'll fit in place of my old one)

I use a few of these... Only nitpick I have is the USB cable is non-removable. They don't come with an adapter, but take 5v straight up.
