want to make opinion poll abiout carmp3 /multimedia systems

Hi,
I know this question doesn't really belong here . I wanna make an opinion poll( hope this is right - if not its my dictonarys fault

. Does any service / Tool etc exist to make html pages for such polls or some weinterfaced generators?
I'm a bit courious and I wanna ask the mp3car comunity some questions to do some stats about where the most people are from age etc etc the results have to be anonymous and shown on a html site as bars for ex mp3 in car ==============100% etc..

Anyone an idea? I can't really program html ( only with dreamweavers help
