I am also interested in something like this. I tried to buy an adapter on ebay and ended up gettin screwed on the deal .. man that makes me mad..

The S is a fuse.
The 4.7R is 4.7 ohms, also sometimes written 4R7.

I have no idea how to read that thing. I'm good at constructing electronic stuff (or so I like to think), but I'm unfamilar with most of those symbols.
Anyone feel like taking them time to explain them? I'll buy you a cookie!

Erm, afaics, most of them are. There are just a few that aren't. It's not like they are particularly non standard.
Ok, a quick rundown of what everything is:
The S thing with 5A next to it is a fuse.
The Zigzag things are resistors.
The --| (-- things are polarised capacitors (eg electrolytic).
The --| |-- things are non polarised capacitors (eg, ceramic).
The thing with 16V next to it is a Zener diode.
The UUUU thing is a coil.
The SG3524 is an integrated circuit (chip).
The MBR745 is a diode.
The NDP6051 is a n-channel mosfet.

You need to get the pinout connections for the IC, transistor, diodes, capacitors.
Work out the current/power rating of the coil.
Most resistor are 1/4Watts but watch out for the low value such as 120Ohms or 0.02Ohms...all depends on how much current is being passed. I dont even know if you can just buy a 0.02Ohms, you might need to make one with resistance wire of a known resistance per meter, with the correct power rating.
You need an electrolytics type capacitors or ceramics. Get the radial type for the electrolytic capacitors, it save some spaces. The voltage rating should be above the highest voltage you gonna apply, 35V should be fine.
The fuse can be 20mm 5A type T, anti-surge.
Design your own PCB, etch, drill and then solder the components. 19V at 3A is a lot of power to pass on those copper clad PCB so make sure you make those power lines very thick...specially the grounds. Put some proper power connectors for neatness.
Dont forget to buy some spare parts, its easy to blow them up if you are not careful.
The hardest part of the project is finding shops with the right parts. The rest are very easy.
And dont forget to put the transistor on a heatsink.

Awww, you spoilt it for them. ;-)
The 0.02 Ohm resistor will need to deal with some serious current, but not power (Power = I*I*R and R is very small). None of the other resistors would appear to need to deal with much current.
The MBR745 will also need heatsinking. If you get the surface mount NDP6051, the circuit board is the heatsink and you'll need good airflow and decent copper planes.
